After successful completion of my 12th grade i chose nursing as my career as i had a bias towards caring for the sick from my salad days.So, i felt, through nursing, i could prove myself in rendering service and helping others, as this is one of the best profession which has got tremendous choices in various aspects of service delivery .Today i am proud to be a nurse, as i care for the needy and thereby could fullfill my desire to become a productive citizen to the society. REFLECTIVE JOURNAL-HCNUR 3016(PART E)The critical incident that i taken was from my personal experience which I observed in my recent aged care placement about four months before. In my opinion, the incident was a major issue. It is evident in the incident that the nurse accidently avoided the patient during the medication round. Poor communication is clear in this situation. If she attended the patient at the correct time she can even avoid the situation. She told me that we can come later and check the patient. If we are thinking in another angle this incident negligence of the patient is also clear her. As a student nurse in that situation i become so worried about when the resident seen in the wash room floor and only thing came into my mind in that situation is to inform the Registered nurse by pressing the alert button. In my view i feel that by my action in that situation helped others to know about it and can help the resident.
When i saw the patient on the floor at first sit i become scared and panic because its was my first situation i come across with and i do not know what to do at that time. Moreover i don’t know at that situation that i should called at that time as i was a student nurse. When nurse came and done the observation and neurological observation i observed her what she done. From that situation i understood that we should not be panic at that difficult situation. After the incident i discussed the situation with the nurses and she given me an advice that be brave and positive in a situation like that and that improved my confidence level. I understood the importance of confidence in nursing job. Now i can handle the situation without any fear. If the same situation happens again i won’t be get panic. I checked many books for the management and nursing interventions for the situation. I studied the rules and policies of the age care unit like manual handling, number of assist for a resident. This all would definitely improve my confidence and to some extent i can come across the fear.
After the incident the nurse told me that don’t be afraid in this type of incident because as a nurse in their professional lie we have to come across many situation like this and in addition to it she said when she was starting her professional like she also had many situation like this but through proper training she come across that. If registered nurse is panic and afraid then it reflects in their care towards the patient. She felicitated me for calling for help by pressing the red alarm bell. After all its a unfordable incident for me. By the advices and reflection i got from there made me mo vigilant and confident in my nursing carrier.
As a reflection and as my strength, even though I was panic at that situation I press the red alert button at the right time which i could inform the nurse at the correct time made to reduce further complication. I was able to be a part of the team by helping in doing blood sugar level and neurological examination. As part of my nurse student placement i understand the importance of documentation and management. After that ordeal I came to realize that it is very important to perform my task with the best of my knowledge to avoid and prevent any mishaps which may lead to a serious complication that may put the patients in jeopardy.

In the situation, Registered nurse acted as the leader like taking the decision and assigned different jobs to different staffs. As a student nurse at that time she told me to do the Blood Glucose level. I done it and informed to the nurse. I think that i done a good job in saving the patient as some time the resident may be fainted due to hypoglycaemia. So, i think that even though i was a student nurse i done my role in helping the registered nurse in the treatment and managing the situation wisely.
In the incident, well the patient gone for hypoglycaemia nurse told me to give some lollies which can improve resident’s glucose level. This all are the critical thinking of the nurse in that critical situation. After that she informed the situation to doctor for further clarifications. By the neurological observation done by the enrolled nurse is also a important step because it can give a picture that whether resident has any head injury. By the nurses role in assigning the job help in improving the team work. Communication and confidence are the two key factors in nursing profession. As a nurse, confidence in behaviour shows the professionalism in their carrier. In nursing, confidence is very vital for assessing, planning and evaluating patient care. As i made a good communication skill with the patient which use values like empathy and support which help resident to get reassured.
Good communication, leadership, confidence are the main key factors needed to manage that situation. I understood the importance of confidence in nursing job. As a result, I tried to acquire more confidence with the support of senior colleague and health professionals working with me. Experiences also play an important role in tackling in that particular situation .For instance, if a registered nurse how are was in that particular incident doesn’t have a leadership quality then she cannot assign duty to different staff which affect team work result in poor nursing care So, all these factors go hand in hand. In addition to it, when patient gone hypoglycaemic nurse told me to give lollies this small step taken by the nurse is only through her clinical knowledge.
From the incident as a reflection of it , I came to realisation that we should be vigilant and aware of our duty and task at hand. I realised the importance of skills like communication, confidence, efficiency are very essential in nursing professional. Moreover, team work and communication is very important in treating a patient.Good communication helps us to give confidence in work as a team which automatically provide a good rapport with the patient and staff working together. As a whole this incident reflected me in my life to be a responsible nurse by resolving all problems.

REFLECTIVE JOURNAL-HCNUR 3016(PART C)In my incident i told before the resident was seen unresponsive in the washroom .It can be because of many reasons like low blood pressure, hypoglycaemia etc. In the beginning i was panic because i do not know what to do in a situation like that. It’s a new experience for me as age care facility is new for me as well as it’s my second week as i mentioned. Before I told that my communication was ineffective because when i first went with enrolled nurse for medication rounds we could not find him in his room at that time she told me we can come latter .But if i looked him in the room or in the wash room may be this situation won’t happen. From this situation i understood that proper communication is essential in nursing profession between staff as well as patient.
Though enrolled nurse told me that latter he can have the tablets i did not inform it to the Registered nurse. This also shows my ineffective communication. It affects the patient’s life, as a situation can avoid by my effective communication. It would also affect other patients circuitously as well, would affect quality of care, productivity and job fulfilment. As a team work communication is very essential otherwise it may affect the life of a patient.
When we coming to the nurse’s role in the incident in my opinion she done a good job. Firstly, she told me do not be panic and fall of patient is common in the age care setting. She makes the situation calm and cool that make others relax to do the job. This shows her leadership quality. Secondly, she given or divided job to each staff for instance, told enrolled nurse to take the obs and told me to do the BGL .This will enable to improve the staffs to work as a team. I reck on ,that by her experience she got these qualities. Work experience is a fantastic way to gain an insight into a career in nursing. It can also be a valuable way of getting some confidence and experience of caring for people. Work experience is a great way of seeing what it’s like working in the health service and our commitment in the nursing job. She may already comes across same situation before.
When i analysing myself as a nurse, if i were in that situation i will do same as she done .In an age care setting, enrolled nurses and personal carers all have the same responsibility in patients safety .In that incident become panic in the beginning but after the advice by the registered nurse my confidence increased and i become a part of that health team. All the staff helped me to do my duty as a student nurse to manage that situation. As a whole by this incident i could now manage this type of situation independely. In that situation, as a student nurse i done BGL and informed nurse about that which helped them to proceed the treatment further. In addition to it ,as my strength, even though I was panic at that situation I press the red alert button at the right time which i could inform the nurse at the correct time made to reduce further complication. REFLECTIVE JOURNAL-HCNUR 3016(PART B) RESUBMISSIONI have learned from my personal experience that even a small negligence is pivotal for the survival of a patient’s life. It is the important duty of the nurse to be alert and conscious in their professional life as they are dealing with life. I understood that it is very vital to deliver my chore with the best of my knowledge to avoid any further complications which even may lead to put patient’s life in danger.
From this incident I erudite a lot of things which would help me to improve my professional nursing life. Primarily, I realized the importance of competent skills such as confidence, communication, efficiency which are essential in the nursing field. As a positive thinking of the incident I realized that I done a positive step by informing others by pressing the assist button. Moreover, I am more over confident of myself in my nursing experience I had in nursing profession.
On the other side, with my effective communication skills I could convince the patients. I can even give more psychological support to the patent after the incident. It all shows my ineffective communication. As a professional nurse communication between co-workers and patients are very essential.

She managed the situation well. She arranged her task to different persons including me, enrolled nurse etc. She started the obs informed doctor it all makes the patients situation better. Her effective critical thinking behavior made me to think like a leader in that hectic situation and brought my confidence level better.
As a leader, I would plan the duty well like assess the situation of the patient. Firstly, assign duty to my colleagues that makes the situation settled. I may use my decision making skills to handle the situation, like informing the doctor and starting the neurological assessment. Moreover, I would do the documentation updated that help other staffs to know the details of the patient.
In my future nursing carrier even for the nursing placement I will use all the small things to progress my communication skill which automatically improve my confidence level to a greater extent. By this I can eliminate many of the communication associated care mistakes. By these reflections enables me to identify my flaws in my nursing skills which are needed for developing competency and can recover my efficiency in nursing practice. Moreover, I feel that by these reflections I can be more vigilant in my duty in future that will make me a perfect professional nurse.

REFLECTIVE JOURNAL-HCNUR 3016(PART A) RESUBMISSIONThis is from my personal experience which I observed in my recent aged care placement about four months before. It’s an aged care home of around 70 residents. It’s my second week of placement I was working with the Registered Nurse on duty on one of my early shift. As usual after the hand over everything was going fine as all the staffs were busy with their residents assigned and their normal routine.
Around 8 am the registered nurse told me to be with the Enrolled Nurse (medication endorsed) for medication rounds. We started our usual medication round. I was keen watching the way she administering the medication. When we enter into one of the resident named Mr. Smith, 55 years room we couldn’t find him in his bed. The enrolled nurse told me that he would have taking the shower so we can come latter. After the rounds she told me to check Mr Smith. When I came back their he was not in the bed. I knocked the washroom door, No response. I opened the door I found he was on the floor unconsciously. I become so panic because it’s a new experience for me. Without second thought I pressed the red alarm bottom. Every one rushed into the room including Registered nurse. The Registered nurse told don’t be panic and we moved him to bed and done the observation and she told enrolled nurse to start the observation chart and asked me to check the Blood Glucose level of the resident and it was low and I informed her about it ,told me to give him some lollies. The doctor was informed and he came and assessed and said he was fine. Addition to it, doctor advised that as he had a fall he want to be in a half an hour neurological assessment.
From the above incident which took place, I came to realisation that we should be vigilant and aware of our duty and task at hand. I realised the importance of skills like communication, confidence, efficiency are very essential in nursing professional. In reflection, now I understood that we should not be panic in this type of emergency situation and be professional in all the steps we take. I observed the process the nurse done throughout the incident and it makes me more confident.

I have learned from that incident that every small detail is vital for the survival of a patient and an important duty of a nurse. After this incident I learned my mistake and it helped me to be alert and vigilant during my nursing life and always show important to even the smallest of detail. Not but the least, as a nurse, after that incident I learned that I’ve got be very alert and confident during my duty and perform all my duty to the utmost professionalism. Where I came to realise life is precious and any small mishap may put it in jeopardy in medical career.
